DESCRIPTION

Glipotec-MP2 Tablet (Glimepiride 2 mg + Pioglitazone Hcl 15 mg + Metformin Hcl 500 mg (ER) belongs to a class of medicines called anti-diabetic drugs used in the treatment of type 2 diabetes, especially in patients whose blood sugar levels are not controlled by diet and exercise alone. Type 2 diabetes is a chronic (long-lasting) condition that affects the way our body processes glucose. People with type 2 diabetes either do not produce enough insulin or the insulin produced is unable to perform its function in the body (insulin resistance).

MECHANISM OF ACTION

Glimepiride is a sulfonylurea that stimulates the pancreas to release insulin by binding to specific receptors on pancreatic beta cells. This leads to increased insulin secretion, helping to lower blood glucose levels.

Pioglitazone is a thiazolidinedione that enhances insulin sensitivity by activating peroxisome proliferator-activated receptor-gamma (PPAR-γ) in adipose tissue, muscle, and liver. This activation increases the transcription of insulin-responsive genes, improving the body's response to insulin, enhancing glucose uptake in peripheral tissues, and reducing hepatic glucose output.

Metformin is a biguanide that primarily decreases hepatic glucose production and improves insulin sensitivity in peripheral tissues by activating AMP-activated protein kinase (AMPK), which plays a role in cellular energy homeostasis. It lowers blood glucose levels by reducing hepatic gluconeogenesis and enhancing peripheral glucose uptake, without increasing insulin secretion.

Metformin requires cautious use in patients with impaired renal function to avoid lactic acidosis, and liver function should be periodically assessed due to potential effects from both Pioglitazone and Metformin.
