DESCRIPTION

 

 

Merocef-SB Inj. (Meropenem 1 gm + Sodium 90.2 mg + Sulbactum 500 mg) consist of a composition of (Meropenem, Sodium & Sulbactam) belongs to the group of medications called anti-infectives used to treat bacterial infections. Bacterial infections are caused by the multiplication of harmful bacteria inside or on the body. These harmful bacteria produce chemicals known as toxins, which can damage tissue and make you sick. Symptoms of bacterial infection vary depending on the organ affected by bacteria. Mercef SB Injection contains Meropenem and Sulbactam. Meropenem is a type of antibiotic. Mercef SB Injection (Meropenem, Sodium & Sulbactam) works by preventing the formation of the bacterial protective covering, which is required for bacterial survival. Sulbactam is a beta-lactamase inhibitor that reduces resistance and improves Meropenem antibacterial activity.

 

 

 

 

INDICATION

 

 

1. Intra-Abdominal Infections: Peritonitis (infection of the abdominal lining) and other severe intra-abdominal infections are often caused by multiple bacteria, including E. coli, Klebsiella, Enterobacter, and anaerobes. Merocef-SB Inj. (Meropenem 1 gm + Sodium 90.2 mg + Sulbactum 500 mg) combination of Meropenem and Sulbactam is highly effective in treating these infections due to Meropenem’s broad spectrum and Sulbactam’s ability to protect against beta-lactamase-producing bacteria that might otherwise be resistant to Meropenem alone.

 

 

2. Respiratory Tract Infections: Hospital-acquired pneumonia (HAP), ventilator-associated pneumonia (VAP), and other lower respiratory tract infections caused by multidrug-resistant organisms (e.g., Pseudomonas aeruginosa, Acinetobacter baumannii) can be effectively treated with Merocef-SB Inj. (Meropenem 1 gm + Sodium 90.2 mg + Sulbactum 500 mg) combination. Meropenem is particularly effective against gram-negative organisms, including multidrug-resistant Enterobacteriaceae and Pseudomonas, while Sulbactam provides an additional barrier against beta-lactamase-producing pathogens.

 

 

3. Urinary Tract Infections (UTIs): Complicated UTIs, including pyelonephritis (kidney infection) and cystitis (bladder infection), caused by resistant organisms such as E. coli and Klebsiella can be treated with Meropenem. The addition of Sulbactam protects against beta-lactamase-producing organisms, enhancing Meropenem’s efficacy, especially in severe or hospital-acquired cases.

 

 

4. Skin and Soft Tissue Infections: Complicated skin and soft tissue infections (cSSTIs), such as cellulitis, abscesses, or wound infections, especially in patients with diabetes, immunocompromised individuals, or those with surgical or traumatic wounds, can benefit from this combination. Merocef-SB Inj. (Meropenem 1 gm + Sodium 90.2 mg + Sulbactum 500 mg) covers a wide range of gram-positive, gram-negative, and anaerobic bacteria that often complicate these infections.

 

 

5. Bloodstream Infections (Septicemia): Septicemia or bloodstream infections are critical conditions that can arise from any infected tissue and rapidly spread throughout the body. Merocef-SB Inj. (Meropenem 1 gm + Sodium 90.2 mg + Sulbactum 500 mg) combination is used for gram-negative sepsis, particularly with organisms such as Klebsiella, Pseudomonas, and Acinetobacter. Meropenem's broad-spectrum activity and Sulbactam’s protection against beta-lactamase-producing organisms are crucial in treating this life-threatening condition.

 

 

6. Gynecological Infections: Endometritis (infection of the uterine lining), pelvic inflammatory disease (PID), and other severe gynecological infections can be caused by a combination of aerobic and anaerobic bacteria. Merocef-SB Inj. (Meropenem 1 gm + Sodium 90.2 mg + Sulbactum 500 mg) combination is effective in treating polymicrobial infections, which are common in these conditions.

 

 

7. Meningitis (In some cases): Meropenem can be used for the treatment of meningitis caused by susceptible gram-negative bacteria, particularly in patients who have been exposed to multidrug-resistant organisms. 8. Complicated Infections in Immunocompromised Patients: Neutropenic fever or infections in immunosuppressed patients (e.g., cancer patients undergoing chemotherapy, organ transplant recipients, or patients on immunosuppressive therapies) are often caused by multidrug-resistant organisms. Meropenem provides broad-spectrum coverage, while Sulbactam enhances its effectiveness against resistant bacteria. Merocef-SB Inj. (Meropenem 1 gm + Sodium 90.2 mg + Sulbactum 500 mg) combination can be used as empiric therapy until the exact pathogen is identified.

 

 

9. Bone and Joint Infections: Osteomyelitis (bone infection) and septic arthritis may require aggressive antibiotic therapy to penetrate bone tissue and eliminate infection. Meropenem is effective against a broad spectrum of bacteria that cause these infections, including Staphylococcus aureus and Pseudomonas aeruginosa, while Sulbactam helps prevent resistance from beta-lactamase-producing organisms. Merocef-SB Inj. (Meropenem 1 gm + Sodium 90.2 mg + Sulbactum 500 mg) is helpful for bone and joints.

 

 

 

 

 

MECHANISM OF ACTION 

 

 

  • Meropenem (1 gm): Meropenem is a carbapenem antibiotic that works by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) in bacterial cells, disrupting cell wall formation, which leads to cell death. It has broad-spectrum activity, covering gram-positive, gram-negative, and anaerobic bacteria.

 

  • Sulbactam (500 mg): Sulbactam is a beta-lactamase inhibitor that prevents bacterial enzymes (beta-lactamases) from breaking down beta-lactam antibiotics like Meropenem. This extends the activity of Meropenem, especially against beta-lactamase-producing bacteria that would normally be resistant to Meropenem.

 

  • Sodium (90.2 mg): Sodium is included in the formulation primarily as a stabilizer and to ensure the correct osmotic balance, allowing the drug to be properly absorbed and delivered into the bloodstream.

DRUG INTERACTIONS

 

 

 

Drug-Drug Interactions: Merocef-SB Inj. (Meropenem 1 gm + Sodium 90.2 mg + Sulbactum 500 mg)may interact with medicines used to treat bipolar disorder (e.g. valproic acid/ sodium valproate), medicines used to treat gout (e.g. probenecid), and oral anticoagulant medications (e.g. dabigatran, apixaban).

 

Drug-Food Interactions: Alcohol may increase the side effects of Merocef SB Injection consist of (Meropenem, Sodium & Sulbactam). Hence, it is advised to limit the intake of alcohol while using Merocef-SB Inj. (Meropenem 1 gm + Sodium 90.2 mg + Sulbactum 500 mg).

 

Drug-Disease Interactions: People with colitis (inflammation in the intestine's lining) and mononucleosis (viral infection with sore throat and fever). Should use Merocef SB Injection consist of (Meropenem, Sodium & Sulbactam)  with caution in patients with severe kidney impairment, and severe diarrhoea should consult the doctor before taking Merocef-SB Inj. (Meropenem 1 gm + Sodium 90.2 mg + Sulbactum 500 mg).

 

 

 

 

PRECAUTIONS

 

 

Merocef-SB Inj. (Meropenem 1 gm + Sodium 90.2 mg + Sulbactum 500 mg) should be used with caution in patients with renal impairment, requiring dose adjustments and regular monitoring of kidney function. Mercef SB Injection (Meropenem, Sodium & Sulbactam) may also increase the risk of seizures, especially in those with a history of seizures or severe renal dysfunction. Mercef SB Injection (Meropenem, Sodium & Sulbactam) combination can cause Clostridium difficile-associated diarrhea (CDAD), so monitoring for gastrointestinal symptoms is important. Pregnant women should only use it if absolutely necessary, while breastfeeding mothers should consult their doctor before use. Caution is also advised in patients with a history of allergic reactions to beta-lactam antibiotics, and blood counts should be monitored for potential hematological changes. Lastly, drug interactions, particularly with Probenecid or Valproic acid, should be carefully managed.
