DESCRIPTION
Tozic Injection (Cefepime 1 gm + Tazobactam 125 mg) is a broad-spectrum intravenous antibiotic used to treat serious bacterial infections, especially those caused by multi-drug resistant organisms. Cefepime, a fourth-generation cephalosporin, targets both gram-positive and gram-negative bacteria, including difficult-to-treat pathogens like Pseudomonas aeruginosa. Tozic Injection (Cefepime 1 gm + Tazobactam 125 mg) works by inhibiting bacterial cell wall synthesis, leading to bacterial cell death. However, some bacteria produce beta-lactamase enzymes that degrade cefepime, which is where tazobactam, a beta-lactamase inhibitor, comes in to protect cefepime from breakdown. This combination enhances cefepime's effectiveness, especially against beta-lactamase-producing bacteria. Tozic Injection is used for infections like pneumonia, complicated urinary tract infections, intra-abdominal infections, and skin infections. It is typically administered in a hospital setting under close medical supervision.
INDICATION
Complicated Urinary Tract Infections (UTIs): Tozic Injection (Cefepime 1 gm + Tazobactam 125 mg) used to treat severe or complicated urinary tract infections, including pyelonephritis, caused by susceptible organisms.
Pneumonia: Tozic Injection (Cefepime 1 gm + Tazobactam 125 mg) is effective in the treatment of both community-acquired pneumonia (CAP) and hospital-acquired pneumonia (HAP), including those caused by resistant pathogens like Pseudomonas aeruginosa.
Complicated Intra-abdominal Infections: Tozic Injection (Cefepime 1 gm + Tazobactam 125 mg) treats infections such as peritonitis, abscesses, or infections following abdominal surgery, caused by a broad range of bacteria.
Skin and Soft Tissue Infections: Tozic Injection (Cefepime 1 gm + Tazobactam 125 mg) used for the treatment of complicated skin and soft tissue infections (SSTIs), including those involving resistant organisms.
Febrile Neutropenia: Tozic Injection (Cefepime 1 gm + Tazobactam 125 mg) provides broad-spectrum coverage to prevent or treat infections in neutropenic patients (patients with a low white blood cell count, typically due to chemotherapy).
Sepsis: Tozic Injection (Cefepime 1 gm + Tazobactam 125 mg) may be used in the treatment of sepsis or septic shock caused by susceptible gram-negative or gram-positive organisms, especially in critically ill patients.

MECHANISM OF ACTION
Cefepime (1 gm): Cefepime is a fourth-generation cephalosporin antibiotic. It works by binding to penicillin-binding proteins (PBPs) on the bacterial cell membrane, which are essential for the final steps of peptidoglycan synthesis in the bacterial cell wall. This inhibition disrupts cell wall formation, weakening the structure and leading to bacterial cell lysis and death. Cefepime has a broad spectrum of activity against both gram-positive and gram-negative bacteria, including difficult-to-treat organisms like Pseudomonas aeruginosa.
Tazobactam (125 mg): Tazobactam is a beta-lactamase inhibitor, which works by binding to and inactivating beta-lactamase enzymes that some bacteria produce to degrade beta-lactam antibiotics like cefepime. Beta-lactamases break open the beta-lactam ring in the antibiotic, rendering it ineffective. By inhibiting these enzymes, tazobactam protects cefepime from bacterial resistance mechanisms, thereby extending its antimicrobial activity against beta-lactamase-producing bacteria.
DRUG INTERACTIONS
Drug-Drug Interactions: Tozic Injection (Cefepime 1 gm + Tazobactam 125 mg) may interact with aminoglycoside antibiotics (streptomycin, neomycin, and gentamicin) as it may cause severe nephrotoxicity (damage to the kidney). It may also interact with a medicine used to treat gout (probenecid).
Drug-Disease Interactions: Tozic Injection (Cefepime 1 gm + Tazobactam 125 mg) should be used with caution in patients with kidney problems, gastrointestinal problems, especially colitis (inflammation of the large intestine) and diabetes.

PRECAUTIONS
Tozic Injection (Cefepime 1 gm + Tazobactam 125 mg) should be used with caution in patients with a history of allergic reactions to beta-lactam antibiotics, as severe hypersensitivity reactions, including anaphylaxis, may occur. Dose adjustments are required in renal impairment to avoid toxicity, and renal function should be closely monitored. The combination can also cause neurotoxicity, including seizures, particularly in patients with impaired kidney function. Prolonged use may increase the risk of Clostridium difficile-associated diarrhea and superinfections. Care should be taken when used with nephrotoxic drugs or probenecid, as interactions may affect drug levels or increase kidney damage.
